The cave is Dunald Mill Hole, in Lancashire, a much engraved curiosity, painted by (amongst others) the Liverpool Pre-Raphaelite James Campbell (his The Dragons Cave, c 1854, Walker Art Gallery). Brewtnall had a keen interest in folk tales and ballads, which often provided the subject matter for his pictures. This picture was exhibited in America in 1893.
